2006-2007 Registration Information

Registration for CYAC swim team is ongoing until enrollment limits are reached.  New swimmers are welcome to attend two weeks of practice free.  Please contact Head Coach Don Remaly for more information about our free two-week trial option.

All the forms required to register your swimmer(s) with the CYAC swim team can be found below:

• Swim Team Membership Application (New)
• CYAC Code of Conduct
• Health Info and Parent Agreement Form (New)
• Parent Volunteer Form (New)

Please view and print out a copy of each document, fill in the required information, and mail, fax or bring to:

• Directions to Piedmont Family YMCA Office:
  • Take Interstate 64 to Charlottesville – 29 North/250 By-Pass exit.
  • Exit the 250 By-passs/29 N at Barracks Road.
  • Turn left onto Barracks Road.
  • Go straight in the right-hand lane to Georgetown Road (next light).
  • Turn right and follow to the next light at Hydraulic Rd.
  • Turn right onto Hydraulic and go to the next light (left lane).
  • Turn Left onto Commonwealth Drive.
  • Continue on Commonwealth Drive 1 light.
  • Take a right onto Westfield Road.
  • The Piedmont Family YMCA office is on the left, just after the Adelphia office.
